Please tell us how rehabilitation is organized in the city.

— The development and optimization of the medical rehabilitation system is one of the priority tasks of Moscow healthcare. The main goal of medical rehabilitation is to reduce disability and improve the quality of life of patients with diseases of the central nervous system, those who have undergone injuries and surgeries on the joints, and patients with cardiovascular pathology.

The relevance of the rehabilitation issue is due to the fact that there is currently a trend toward growth (on average by 3% compared with 2014) in the number of patients with acute diseases requiring medical rehabilitation, and above all this is an increase in the number of patients with acute cerebrovascular accidents by 5% and an increase in the number of patients with musculoskeletal injuries by 3%.

In Moscow, medical rehabilitation is carried out in three stages:

Stage I is carried out during the acute period of the disease in intensive care units, intensive therapy units, and specialized hospital departments;

Stage II is carried out in the early recovery period of the disease in rehabilitation hospitals;

Stage III is carried out in the late recovery period in outpatient and polyclinic settings, in rehabilitation departments and offices, at home, and in sanatorium-resort organizations. In order to optimize care for patients in medical rehabilitation, we are improving the three-stage rehabilitation system, striving to organize interaction between medical organizations at various stages of medical rehabilitation, and to ensure patient routing to guarantee continuity and succession in the provision of this type of care.

The Moscow Healthcare Department attaches great importance to the development of medical rehabilitation, which is why in 2015 I, as the chief freelance specialist, twice reported on the implementation of the main areas of healthcare modernization in Moscow at a meeting of the project office, designed to identify problems in the development of individual healthcare areas and develop ways to solve them.

— What are the main problems in the development of medical rehabilitation?

— I would not call them problems; they are rather difficulties that we are addressing. First of all, as I have already said, it is necessary to ensure the accessibility, continuity, and succession of medical rehabilitation care. For this purpose, the clinics of the Moscow Scientific and Practical Center for Medical Rehabilitation, Restorative and Sports Medicine work closely both with hospitals and with outpatient and polyclinic medical organizations, as well as with the chief freelance specialists of the Moscow Healthcare Department in neurology, traumatology and orthopedics, and cardiology. The patients admitted to us are quite seriously ill and often need outside help. Branch No. 3 — the Multifunctional Clinic of Medical Rehabilitation — provides medical rehabilitation for patients who have suffered an acute cerebrovascular accident, patients after injuries and surgeries on the locomotor system, as well as patients with diseases of the joints and peripheral nervous system. A patient with stabilized vital functions and rehabilitation potential is provided with medical rehabilitation in full.

Another issue is patients’ awareness of routing and the possibilities for further rehabilitation after acute illnesses. Patients need to be better informed about the possibility of rehabilitation, its timing, and its limitations, and this is provided through information on the websites of the Moscow Healthcare Department and the Scientific and Practical Center for Medical Rehabilitation, Restorative and Sports Medicine, as well as during consultations with our specialists.

The issue of financing medical care in medical rehabilitation remains rather controversial. The fact is that until 2013, funding for medical rehabilitation care was provided from the Moscow city budget. The transition of Moscow healthcare to the capitation financing principle within the compulsory medical insurance system and the difficult economic situation show that there is not enough money for medical rehabilitation. Indeed, restorative treatment requires more time, the use not only of medications but also of expensive high-tech equipment and highly qualified personnel. Often, recovery after a stroke is the result of a team of specialists working, moreover, in a “manual mode.” MGFOMS and DZM are doing a great deal to provide financial support for medical rehabilitation. We are realists and understand that compulsory medical insurance cannot fully cover the financial costs of rehabilitation.

— Alexander Nikolaevich, you are a scientist and manager with extensive work experience; what solutions do you see in this situation?

— In our opinion, it is necessary to intensify and actively develop public-private partnership in the field of restorative medicine and medical rehabilitation in the country, which will allow this important area to be as effective as possible. It is exactly this promising model that is supported by Moscow Deputy Mayor in the Moscow Government for Social Development L.M. Pechatnikov, and it will be implemented in the form of a pilot project of a multidisciplinary center for medical rehabilitation and restorative medicine.

— Which achievements of your service do you consider important in the past year?

— First of all, I believe that we have created a medical rehabilitation system in Moscow, and it is working. Over the past year, 13,193 people underwent rehabilitation at the MNPC for Medical Rehabilitation, Restorative and Sports Medicine. In total across the city — 16,864 people. As can be seen from the figures above, the MNPC for Medical Rehabilitation, Restorative and Sports Medicine provides 78% of care in this area. We were able to achieve such results by increasing the number of bed-days per year, bed turnover, and, of course, without compromising the quality of care provided to patients.

I consider it important that rehabilitation issues are also being addressed in pediatric practice. Professor M.A. Khan, a specialist well known both in the capital and in Russia, oversees children’s rehabilitation in the city of Moscow.

Last year we organized training for specialists, advanced training in medical rehabilitation and restorative treatment in the form of monthly seminars, in addition to month-long courses designed for 144 hours. In 2015, 250 Moscow doctors of various specialties completed training. This is extremely important, as it increases access to medical rehabilitation care and the qualifications of the doctors who provide it. Also serving the goal of improving the quality of medical rehabilitation is the preparation of methodological materials; over the year, 9 methodological guides for doctors were published.

We held the 1st International Congress on Physiotherapy, Therapeutic Exercise, and Rehabilitation, which brought together more than 500 specialists from across Russia. Every year, staff from the center participate in the “Health of the Capital” forum, where Moscow doctors exchange experience, and scientists present advanced technologies. In 2015, our symposium at the forum was dedicated to rehabilitation in pediatric practice.

We are constantly trying to move forward. Thus, our scientists, together with one of the major companies, developed a registry for medical rehabilitation, which was done for the first time in the country. The implementation of the registry will make it possible to account for every patient in need of medical rehabilitation, assess the volume of care provided and its effectiveness, which is very important for the further development of our field.

It should be noted that the State Program for the Development of Healthcare of the Russian Federation until 2020 and subprogram 5, which we implement: “Development of Medical Rehabilitation and Sanatorium-Resort Treatment,” have clear indicators broken down by year. I can state with satisfaction that, despite the difficulties, Moscow healthcare has met its target indicators over the past three years and will certainly continue to implement the state program in full.

Elena Nikolaeva

http://ktovmedicine.ru/

[2

post-1050","post","type-post","status-publish","format-standard","hentry","category-news
["GET
